Ohio Casino Gaming Employee License
All persons participating in any capacity in a excursion gambling boat/gambling structure/racetrack casino are required to obtain an occupational license from the Commission. A license is valid for three calendar years. All licenses are temporary until the completion of required background investigations.
- All casino gaming employee licensees and applicants must submit to the Ohio Casino Control Commission the required information per Ohio Administrative Code, chapter 3772-8-04. You should fill out the Duty to Update form if: You have changed your name, address or primary phone information.
- Licenses and registrations of casino key employees and gaming employees issued pursuant to this article shall remain valid for five years unless suspended, revoked or voided pursuant to law.
- Casino gaming is a relatively new industry in Ohio, having been approved by voters in an amendment to the Ohio Constitution in 2009. The Ohio Casino Control Commission was created in 2011 to regulate the four casinos authorized by the constitutional amendment, located in Cincinnati, Cleveland, Columbus, and Toledo.
- CASINO LICENSING: Licensure is the cornerstone of the regulatory system. Licenses are required of casino owners and operators, casino employees, and companies that do business with casinos in order to ensure that those involved with this industry meet the statutory requirements of good character, honesty and integrity and to keep the New Jersey casino industry free from organized crime.
Each applicant must complete and sign an application form prescribed and published by the Commission and pay a license fee set by the Commission, as well as any other associated fees that may apply.
License Fee: $10 for Non-Gaming Licenses, $20.00 for Gaming Licenses
IRS 4506T Tax Form Fee: $6.00. This fee is assessed by the IRS to process the form that is included in the background investigation for only Class A and Class C licenses.
The Ohio commission bills casinos for the licenses and it is up to the casino whether to require reimbursement from employees. Horseshoe Casino Cleveland has about 1,100 licensed employees.
Fingerprint Fee: $42.00. The Commission shall charge the applicant the fee set by the Federal Bureau of Investigation and by the Iowa Department of Public Safety, Division of Criminal Investigation to cover the costs associated with the search and classification of fingerprints.

2. After such investigation, the commission may either deny the application or grant a license to an applicant whom it determines to be qualified to hold such license.
3. The commission shall have the authority to deny any application pursuant to the provisions of this article following notice and opportunity for hearing.
4. When the commission grants an application, the commission may limit or place such restrictions thereupon as it may deem necessary in the public interest.
5. After an application for a casino key employee license is submitted, final action of the commission shall be taken within ninety days after completion of all hearings and investigations and the receipt of all information required by the commission.
6. Licenses and registrations of casino key employees and gaming employees issued pursuant to this article shall remain valid for five years unless suspended, revoked or voided pursuant to law. Such licenses and registrations may be renewed by the holder thereof upon application, on a form prescribed by the commission, and payment of the applicable fee. Notwithstanding the forgoing, if a gaming employee registrant has not been employed in any position within a gaming facility for a period of three years, the registration of that gaming employee shall lapse.
8. The commission shall establish by regulation appropriate fees to be paid upon the filing of the required applications. Such fees shall be deposited into the commercial gaming revenue fund.
